A harrowing case of alleged negligence has emerged from a nursing home in Pinellas County, Florida, where the personal representative of a deceased resident’s estate is seeking justice. On November 12, 2025, Raymond Feind filed a complaint in the Circuit Court of the 6th Judicial Circuit against Oakpark Operations LLC, Aston Healthcare LLC, and Citadel Consulting Group LLC. The complaint accuses these entities of failing to provide adequate care to Carrie Feind during her residency at their facility.

Raymond Feind alleges that his late wife, Carrie Feind, suffered severe neglect while residing at the defendants’ nursing home. According to the complaint, filed by Senior Justice Law Firm on behalf of Mr. Feind, the defendants failed to meet their obligations under Chapter 400 of Florida Statutes. This statute sets forth regulations for operating nursing homes in Florida. The plaintiff contends that the facility’s staff did not provide necessary supervision and assistance for Mrs. Feind’s daily activities, which led to preventable injuries such as skin breakdowns and falls resulting in a hip fracture and a comminuted displaced left intertrochanteric femur fracture on January 9, 2025.

The lawsuit outlines multiple failures by the defendants including inadequate staffing levels, insufficient training for employees, failure to maintain cleanliness and safety within common areas, and neglecting to inform family members about significant changes in residents’ conditions. “Defendants owed a duty to provide reasonable care,” states the complaint, emphasizing that these failures directly contributed to Mrs. Feind’s deteriorating health and eventual death on January 19, 2025.

Mr. Feind seeks compensation for wrongful death damages under Florida’s wrongful death statute. These damages include medical bills incurred before Mrs. Feind’s passing, funeral expenses, loss of net accumulations for her estate, and compensation for mental pain and suffering endured by her survivors due to her untimely death. Additionally, if it is determined that Mrs. Feind’s injuries did not cause her death but instead led to prolonged suffering before she passed away naturally or from unrelated causes—her estate seeks survival damages covering economic losses like medical expenses as well as non-economic damages such as pain and suffering experienced prior to her demise.

The case is represented by William J. Sarubbi I Esq., from Senior Justice Law Firm located in Boca Raton, Florida. It has been assigned Case Number: 25-006387-CI.
